Eligibility Criteria
To be eligible for home care, a patient must:
Have on-going medical supervision and orders from a physician.
Live in a reasonably safe home environment with a responsible person available to assist if necessary.
Require home health care in the home on a part-time, intermittent basis.
Medicare clients must be homebound or require the assistance of another to leave the home.
